Whom We Serve

Supportive Homecare Options, Inc. (SHO) is a contracted provider for members of the Family Care program. SHO provides services exclusively to members enrolled in Family Care through My Choice Wisconsin. My Choice Wisconsin is a managed care program that provides long-term care services for frail elders, adults with chronic illnesses or disabilities preventing them from being able to effectively carry out their daily activities. When a member is enrolled in services with SHO, SHO works to hire and train their preferred caregiver to provide in-home personal care and supportive homemaking services. If the member does not have a preferred caregiver, we work to find them a worker to provide supportive care. SHO works to assist clients in remaining safe and independent in their own home or apartment. If you or your loved one are seeking services from SHO, please contact your case manager to see if you are eligible to participate in our services. Once SHO receives an assessment and authorization for services, SHO can begin the hiring process for your caregiver and set up services.


Services

We provide two types of services:

  • Supportive Home Care (homemaking) services generally encompass: basic cleaning and organizing throughout the home, meal preparation, laundry, and/or shopping.

  • Attendant Care (hands-on client care) services generally include assisting a client with: bathing, dressing, grooming, toileting, mobility/transfers, and/or feeding.

Each client’s needs are different. The type of services, as well as the frequency and length of services are set up by the Interdisciplinary Team, which consists of the member/client, their Case Manager, their Nurse Case Manager and sometimes a Power of Attorney. The Interdisciplinary Team assess the member needs and selects services to support the member. It is up to the Interdisciplinary Team to select a service provider and authorize the type, frequency and duration of services provided. SHO uses the assessment and authorization to set up a caregiving schedule and support the client. SHO does not choose how many hours the client receives services, this information is coordinated by the Interdisciplinary Team and the client.


SHO Call Center

The SHO Call center (414-475-5356) is operated by a dedicated team of Care Coordinators and Timesheet Technicians. This team coordinates and communicates worker and client schedules, and verifies the services for billing and payroll. When calling into the office, if you call cannot be immediately answered, you must leave a complete message including: your name, the phone number where you can be contacted, and a brief description for the reason of your call, and we will return your call. We cannot call you back if you do not leave a voicemail. Due to the high call volume we receive, we kindly ask that you leave a message and wait for a return call.

This team is available to clients and workers to assist with tasks such as, but not limited to:

  • Reporting anticipated absences and/or tardies.

  • Establishing or changing work schedule (Note: changes must be made in advance to working the schedule).

  • Receiving updated duty sheets.

  • Requesting more work hours and/or clients.

  • Notifying the office of work incident and/or injury

  • Reporting any safety hazards in the work environment and/or for client’s safety

  • Reporting client-related concerns in terms of health, safety, welfare and/or suspicion of client abuse or neglect

  • Reporting worker absences and/or concerns

  • Reporting client change of conditions, client falls, and/or hospitalization.
